Assuming you are the named beneficiary, you must fill out the proper claim form for your specific state. These claim forms can be downloaded from the Union Fidelity Life Insurance website. If you do not see your specific state listed, select "All Other States" button for a generic claim form. If you have trouble with the download, the claim forms may be mailed to you directly by calling (800)369-3990.
Besides the standard info you would expect in regards to the decedent and beneficiary, you will also need to have the policy number and dates of issuance. If the policy was a “Term Life Insurance” policy, you will also need to verify that policy was in effect at the time the policy holder passed away.
Finally, these forms must be signed in the presence of a public notary before submitting them.
